Screening Packet
This is the Pre-Admission Screening Packet. Every man who wishes to become a client at Extended Aftercare must complete a screening with a Licensed Chemical Dependency Counselor. The screening is required by law to ensure that Extended Aftercare can properly treat the man. Screenings are done by appointment only and can be done via telephone if the man lives outside of Houston or cannot get to Extended Aftercare for a in-person screening. We encourage men to bring family and loved ones to the screening appointment so they can take a tour of our facility, have their questions and concerns addressed and make a fully informed decision about admission.
Instructions: You you would like to be prepared for the screening, please print this document and read the HIPPA Rights. Sign and date the HIPPA Rights. Continue in the packet and fill out as much information as possible until to get to the signature block. Stop here. When you arrive at your screening appointment a LCDC will fill out the rest of the packet with you. Self Pay Agreement
This is the Self-Pay Agreement. Men who do not qualify for funding through the Veterans Administration, Department of Assistive and Rehabilitative Services, or personal Medical Insurance have the option of funding their own treatment, housing, food and amenities at Extended Aftercare. The cost covers all individual, group and family counseling, chemical dependency education, relapse prevention, 12-step recovery, sober socialization activities, alumni interaction, job quest training, personal and social adjustment classes, as well as all meals, housing, and 24-hour supervision. Other treatment centers can cost upwards of $12,000 for 28 days of treatment while EAI keeps our costs as low as possible to better serve men, and their families, in times of crisis.
Instructions: If a man or his family plans on paying the Self-Pay rate for treatment at EAI the financially responsible party must print, read and sign the Self-Pay Agreement before the man is admitted into Extended Aftercare, Inc. This is the Clinical Menu. This is provided for informative purposes only. As you can see from the Menu, Extended Aftercare does all we can to provide a variety of groups at multiple times of the week to better allow men to balance their treatment with “real world” responsibilities. Family Day Invitation
This is the Family Day Invitation. Extended Aftercare hosts a Family Day of Healing every second Saturday of each month. This afternoon group is facilitated by Family Therapist Deborah Darnell-Drake, LMFT, LCSW and consists of both an educational, teaching component, with discussion around the support of the Alanon and Alateen programs, and, experiential exercises in small group and individual family settings.
